QAP update annual declarations

Each year every CMLTO member self-reports compliance to a series of Quality Assurance declarations during the annual renewal process. Members simply answer Yes or No to each statement. These declarations highlight essential professional practice requirements for safe MLT practice. Every Practising member who answers No is contacted by Quality Assurance (QA) staff to provide an explanation for their noncompliance. Every noncompliance is reviewed by QA staff and may be referred to the Quality Assurance Committee for review.

1. I have complied with the CMLTO Code of Ethics.
2. I have complied with the CMLTO Standards of Practice while working as an MLT.
3. I am competent for practice as an MLT.
4. My Professional Portfolio is up-to-date.

1. I complied with the CMLTO Standards of Practice and Code of Ethics.
2. I am competent to practise as a medical laboratory technologist.
3. I understand that it is my professional obligation to maintain an up-to-date Professional Portfolio.
4. I understand that it is mandatory for Practising members to participate in the CMLTO Quality Assurance Program.
5. I understand that I must submit to the College my self-assessment and learning goals by March 31^st.
