Change of Address or Name

The CMLTO Consolidated Bylaws (PDF) require members to advise the College of any change of address within 30 days of the change.

Any changes to demographic information, employment, education, specialties and certifications, and other registration information may be updated at www.cmlto-onlineservices.com.

Profile changes that require supporting documentation will be reflected on your profile once confirmed by CMLTO. Supporting documentation consists of original documents or copies verified by a guarantor.

Note: To request a name change, copies of two pieces of government-issued identification (driver's license, health card, passport, etc.) that are signed by a guarantor must be submitted. A completed Guarantor's Verification Form (PDF) must also be submitted. This documentation must be mailed to the CMLTO office at:
